Your First Visit

To start care, call our office at 850-678-8048 to schedule an appointment. We will do our very best to get you in our office the same day you contact us or the next day.


Directions and Parking

Our office is conveniently located at 1550 E. John Sims Parkway in Niceville in the Publix shopping center. The shopping center is easily accessible to all surrounding areas of Niceville and we have plenty of parking outside our office. For patients traveling from Crestview, Holt, and Pace, simply take Highway 85 south to the Mid Bay Bridge Connector and exit at the Forest Road exit. Take Forest Road to Rocky Bayou Drive and turn right. Follow the road to the round-a-bout and the Publix shopping Center and Core Chiropractic of Niceville will be on the right. Easy, fast access, and no tolls to pay!

At Core Chiropractic of Niceville, our state-of-the-art office allows you to go paperless and fill out new patient information online from our website. Your information downloads directly to our office computers over a secured network. This allows you to complete your "paperwork" in the comfort of your home from your computer, tablet or smartphone on your schedule.


Consultation and Examination

You can count on our helpful staff to greet and welcome you to our office. Once your consent forms and any outstanding paperwork is completed and we have collected any insurance information, you will have a consultation with Dr. Jacks to discuss your health-related issues and concerns.

Dr. Jacks will perform a chiropractic examination and conduct several specialized postural, neurological, and physical tests.


X-Ray Studies

Your specific condition may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you. X-rays are typically taken on your first appointment using our state-of-the-art digital x-ray machine. Digital x-rays are safer by reducing the amount of exposure, provide better diagnosis detail to determine the exact cause of your problem and they are eco-friendly which eliminates the need for films and processing chemicals. X-rays will help us locate any misalignments, degeneration, and other issues affecting your neck and spine.


Home Instructions

Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.


Report of Findings

We will schedule a convenient appointment time for your follow-up visit. Generally speaking, patients are seen within 1-2 days and will sit down with Dr. Jacks to review a complete report of findings from the examination and x-rays. It is at the second appointment where the patient will be provided with their first chiropractic treatment. This may include spinal adjustments, physical therapies, and/or soft tissue therapies. Additional treatment options for your condition will also be discussed during this time.


Chiropractic Treatment

After your report of findings, you will be given the choice of receiving your first chiropractic adjustment. This will include spinal adjustments as well as adjustments with a specialized instrument performed by the doctor. From this point, our friendly staff will go over the cost of care and assist you with setting up your appointments for future chiropractic treatments as recommended by Dr. Jacks.


Insurance Information

We take most insurance plans, including Medicare. Patients who are receiving corrective care and wish to use their health insurance must present their insurance cards and ID upon their initial visit. We will verify your benefits, check your deductible and verify your co-pay. We will file your claims and we will COLLECT all co-pays at the time of service. You may be responsible for meeting your deductible before you are eligible for benefits and you must pay your co-pay and any other non-covered services your insurance does not cover at time of service. Unfortunately, we cannot offer any specials or discounts on services when you use your health insurance or Medicare.


No Insurance? No Problem!

Many patients do not have major medical health insurance coverage or have insurance plans with extremely high deductibles and limited benefits. In this case, we offer individualized care plans so you can continue your chiropractic treatment.

Good Faith Estimate

If you are NOT filing any insurance, you have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost for non-emergency services. You can ask us for a written Good Faith Estimate. To receive your GFE, you must call our office at least 48 business hours in advance of your appointment to make a request and supply us with your email address. We will email you your estimate 1 day before your scheduled appointment.
If you are a self-pay patient and/or receiving non-covered services, you may use the information below to offically serve as a GFE for our office. This GFE is ONLY for a patient who is NOT filing major medical insurance, Medicare (covered services) or auto Insurance. For all new patients to our office, a chiropractic examination and x-rays will likely be performed on your first visit. For Medicare patients, Medicare considers any chiropractic examination and x-rays as non-covered services. For Tricare patients, Tricare will not cover any chiropractic services.

School or Work Physicals: Self Pay Cost: $ 50.00

Range of Motion Studies: Self Pay Cost: $ 75.00 per region.

For additional information about the Good Faith Estimate you can visit www.cms.gov/nosurprises. The GFE is not a contract and does not require you to obtain the items or services form the provider or facility identified in the good faith estimate. If you receive a bill that is at least $ 400.00 more than your GFE, you can dispute the bill. A written/emailed GFE is only valid for 15 days from the initial date received.
